This Privacy Policy explains how Legacy Family Medicine PLLC (“Legacy Family Medicine,” “we,” “us,” or “our”) collects, uses, discloses, and protects information in connection with legacyfamilymd.com and our general business communications.
1. Information We Collect
Information you provide
We may receive information that you choose to provide when you call or email us, request information, schedule a meet-and-greet, inquire about an employer program, enroll in the practice, or otherwise communicate with us. Depending on the interaction, this may include your name, email address, telephone number, employer or organization, communication preferences, and the content of your message.
Our public website does not ask you to submit medical records or payment-card information directly to Squarespace. Enrollment, patient information, and payment details may be collected through Atlas.md or another clinic-authorized system. Scheduling information may be collected through Google Calendar or another scheduling service. Those services process information under their own privacy terms and, where applicable, agreements with Legacy Family Medicine.
Information collected automatically
When you visit the website, Squarespace and its service providers may automatically receive technical and usage information, such as your IP address, browser and device type, operating system, referring page, pages visited, clicks, scrolling activity, and timestamps. We have disabled Squarespace’s visitor activity log. Analytics and performance cookies are restricted unless you consent through our cookie banner.
Cookies and similar technologies
The website uses necessary cookies and similar storage technologies to operate securely and remember privacy choices. With your consent, Squarespace may also use analytics and performance cookies to help us understand site traffic and activity. You can accept, decline, or manage nonessential cookies through the website’s cookie banner. You can also clear or block cookies using your browser settings, although doing so may affect some site features.
2. How We Use Information
We may use information to:
- operate, secure, maintain, and troubleshoot the website;
- respond to questions and requests;
- provide information about membership, services, scheduling, and employer programs;
- facilitate enrollment and other transactions you request;

- display patient testimonials when we have the appropriate permission or authorization; and
- comply with legal obligations, enforce our rights, and protect people, the clinic, and our systems.
3. Advertising and Retargeting
We do not currently use Meta Pixel, Google Ads remarketing tags, or similar third-party advertising pixels on this website, and we do not currently use website activity for cross-site targeted advertising.
If we introduce retargeting or similar advertising technology in the future, we will update this policy and configure the technology to require any consent required by applicable law. We will not use information from patient portals, enrollment forms, appointment details, medical communications, or other protected health information for advertising or retargeting.
4. How We Disclose Information
We may disclose information as reasonably necessary to:
- Website and technology providers, including Squarespace for hosting, security, video delivery, and consent-based analytics;
- Enrollment and practice-management providers, including Atlas.md, when you choose to enroll or use those services;
- Scheduling and communications providers, including Google services, when you schedule or communicate with us;
- Professional advisers and contractors who support our operations and are subject to appropriate duties or agreements;
- Authorities or other parties when required by law, legal process, or reasonably necessary to protect rights, safety, or security; and
- A successor organization in connection with a merger, acquisition, financing, reorganization, or transfer of all or part of the practice, subject to applicable legal requirements.
We do not sell personal information for money. We do not currently share personal information for cross-context behavioral advertising or targeted advertising.
5. Third-Party Services and Links
The website links to services operated by other organizations, including Atlas.md and Google Calendar. A third party’s collection and use of information is governed by its own privacy policy and terms. We encourage you to review those policies before providing information. Our cookie banner controls Squarespace cookies and supported integrations, but it may not control every technology used on a third-party website after you follow an external link.
6. Retention
We retain website and general business information only for as long as reasonably necessary for the purposes described in this policy, including responding to requests, maintaining business records, resolving disputes, protecting security, and complying with legal, accounting, or regulatory requirements. Retention periods vary based on the type of information, the service involved, and applicable law. When information is no longer required, we take reasonable steps to delete, de-identify, or securely retain it until deletion is practical.
Medical records and protected health information are maintained under separate healthcare record-retention requirements and clinic policies.

We use reasonable administrative, technical, and physical safeguards designed to protect information. No website, email system, transmission method, or storage system can be guaranteed to be completely secure. For medical questions or sensitive patient information, use the clinic’s approved patient communication method rather than ordinary email or a public website.
8. Your Choices and Privacy Requests

- manage nonessential cookies through the cookie banner or your browser settings;
- opt out of marketing email by using an unsubscribe link, when provided, or contacting us;
- ask to access, correct, or delete personal information, subject to applicable law and exceptions; and
- withdraw consent for future processing when our processing is based on consent.

9. Children’s Privacy
The public website is intended for adults, including parents and guardians seeking information about family medical care. We do not knowingly collect personal information directly from children under 13 through the public website. A parent or legal guardian should complete enrollment and communicate with the clinic for a minor as appropriate.
10. Do Not Track and Preference Signals
Some browsers offer “Do Not Track” or other privacy preference signals. Because there is not one universally accepted standard, the website may not respond to every signal. We honor the choices available through our cookie banner and will process legally recognized opt-out preference signals when required by applicable law and technically supported.

If you opt in, Legacy Family Medicine may send recurring marketing and promotional text messages, such as practice updates and occasional offers. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not required to submit a form or receive services.
This SMS program is nonclinical and does not request or accept medical information or protected health information (PHI). Do not send health information by text.
